Corrective Action(s): Manager agreed to use single serve utensils only. All cooking and serving utensils must be washed and sanitized manually. Manual dishwashing discussed with the manager.

Corrective Action(s): Technician was on site on April 26,2019. Technician recommended installing a new dishwasher and new dishwasher was not installed. Install a new dishwasher by July 23,2019.

Note: Facility is on water advisory due to unacceptable Arsenic levels. facility is using bottled water for human consumption and food preparation. Bottled water is purchased from Safeway.
