Observation: OUTSTANDING: Rodent droppings observed in the following locations;
-in the electrical panel in the staff storage room
-around the ice machine
-within cupboard around horseshoe portion of bar
-behind POS and slush machines at the bar
-in the curtained storage area and storage closet
1 dead mouse found by back door into river market (near billiard tables)
Corrective Action(s): Remove droppings, clean and sanitize area using 200 ppm QUATS sanitizer solution

Observation: Rodent entry holes found around the electrical panel
Shredded insulation and cardboard found around ice machine, curtained storage area (on carpet), and under server water station
Holes in walls noted around ventilation in the far storage room (past curtained storage area)
Corrective Action(s): Rodent proof the holes, remove the shredded insulation and cardboard found in areas listed previously, and remove excess equipment from the ventilation and storage room to allow for block entry holes

Observation: Excess equipment in the curtained storage area, storage closet, and ventilation close area preventing adequate cleaning and maintenance of the facility.

Comments

Kitchen area has been cleaned significantly, however, rodent droppings indicating active rodent infestation observed in the following areas;
-under the oven
-on the floor level dry storage shelves
-on shelf behind kitchen entrance door (near dishpit)
Bar areas noted to have rodent droppings must be cleaned with more detail. Rodent droppings still present as indicated in violations noted above. Ensure that a DEEP CLEAN is conducted.
Remove kick board from underneath cupboards at server water station as there is shredded insulation and evidence of rodent harborage in the area.

Operator is hereby required to set up meeting with kitchen manager, bar manager and general manager to discuss future expectations for cleaning and eliminating rodent infestation
