Observation:
- One live cockroach spotted on the wooden base board underneath the 3 compartment sink.
- Dead cockroaches observed inside cabinets at the front bar.
- Flies; unidentified insects observed in the cabinet with water damage.
Corrective Action(s):
- Replace old traps.
- Thorough cleaning required. Check all areas prone to moisture build up and pest activity and ensure adequate maintenance work is done to prevent pest attraction.

Observation:
- Gap observed between the back door and the ground.
Corrective Action(s):
- Install door sweeper to provide complete blockage. Ensure back door is closed at all times when there is no active delivery occurring.

Observation:
- Rodent droppings observed along the wall in the kitchen near the entrance door.
- Biofilm and filth build up on the wooden baseboard in the dishwashing area.
- Mould and moisture build up observed in the cabinet (lower shelf) at the front bar.
- Clutter observed inside the mop sink room.
Corrective Action(s):
Clean and sanitize areas noted above.
Organize miscellaneous items inside the mop sink room. Separate food equipments away from cleaning supplies.

Observation:
- Wooden base board on the floor below 3 compartment sink and dishwasher is prone to moisture damage.
Corrective Action(s):
- Operator stated that they are planning to replace the board with stainless steel material. This base board cannot be effectively cleaned & dried; subject to mould & food debris build up which could lead to pest attraction. Remove the wooden base board and replace it with a material that is non porous and easy-to-clean. Operator to update on the renovation schedule to the undersigned.

Observation:
- Upright freezer in the storage room has ambient temperature of 0C - 1C. It appears the fan unit is not working properly and there are excessive ice build up in the unit.
- Pipe in the lower shelf of the left most cabinet (front bar area) is leaking.
Corrective Action(s):
- Repair the freezer. 1C is too warm for freezer temperature.
- Repair the pipe and ensure there is no leak. Replacement of the whole cabinet may be required due to water damage. If mould cannot be removed from the wooden boards, replace the damaged parts at least.
