Observation: Small inserts/containers of pre-measured dry ingredients were stored on a storage rack, uncovered.
Corrective Action(s): Ensure all food/ingredients are stored properly covered and with secure lids to protect from potential contamination.

Observation: Mouse activity noted (mouse droppings under dishwashing area and in corners of facility/in hard-to-reach areas, and chewed-out walls/foam filling from previously filled holes).
Corrective Action(s): Staff stated pest control company has been servicing on a weekly basis, however have/will be missing some scheduled services due to holidays. Continue with weekly servicing, and ensure to keep copies of invoices/reports onsite for health inspector to review. Remove any droppings found onsite and disinfect affected areas with 1000 ppm bleach solution, and do so on a daily basis. At the end of each day, ensure the entire facility is deep-cleaned and ensure all food is properly covered/protected from contamination. Fill or re-fill any new holes using steel wool and foam, or stainless steel plates.

- Hand washing sinks were properly supplied with hot/cold running water, liquid soap, paper towels.
- Bleach solution for sanitizing food contact surfaces available at 100 ppm.
- Dishwasher achieved 100 ppm chlorine residual, detected at the dish surface after rinse cycle - good.
- Walk-in cooler and pizza prep cooler measured at 4C or colder.
- Walk-in freezer measured at -18C or colder.
- Triple-sink maintained in sanitary conditions. Sink drain stoppers available for use.
- Mop sink maintained in sanitary conditions.
- Foodsafe certified staff onsite.
