Operator stated they are waiting for the rolls to be delivered today. Corrective Action(s): Another roll of paper towel (different size as the dispenser) was placed near the dispenser. Next time, plan ahead to ensure supplies are maintained for hand washing stations. Violation Score: 5

Operator stated they are waiting for the delivery to come. Corrective Action(s): Close the back entrance door when not in use to reduce the risk of pest entry. Back door should only be opened during the delivery. Violation Score: 3
