Observation: Observed cockroaches at the following locations:
- Prep kitchen, behind coolers and freezers
- Dish pit: area where clean and sanitized food contact surfaces are stored, on top of grease trap and near the mop sink area.
- Dry storage area: along the metal railing
- Staff uniform closet: roach droppings
Observed moderate number of house flies throughout the facility. Observed dried rat droppings at the dry food storage area and under the hand sink near the East end of cook line.
Corrective Action(s): - Contact a pest control company for an assessment.
- Clean and sanitize all surfaces (food and non-food contact surfaces).
- Treat the facility as per a pest control company's recommendation.
- Remove rodent droppings.
- Install sticky traps for flies.
- Block gaps and crevices where pest can enter (e.g., gaps in back door).

Observation: Observed a back door with gaps big enough for pest to enter. Sanitation issues noted at hard to reach areas such as behind the cooler, etc.
Corrective Action(s): Any gaps and crevices must be blocked as per your pest control company's recommendation. All food contact surfaces and non-food contact surfaces are to be cleaned and sanitized.

Observation: Walk in cooler and coolers in prep kitchen were measured higher than 4C.
Walk in cooler: 12.9C
Cooler 4: 6.1C
Cooler 2: 8C
Glass cooler: 5.3C

Excessive water leaking from a compressor unit in a walk in cooler. Large pool of water observed under the compressor.
Corrective Action(s): All coolers must be serviced so that it can maintain temperature at or below 4C. Walk in cooler is to be serviced so that water is not leaking from a compressor unit.

Comments

Closure order issued at the time of inspection due to cockroach infestation.
All foods prepared today was voluntarily discarded by the operator.
The operator initiated cleaning procedure at this time.
Hand washing stations are fully equipped with hot and cold running water, liquid soap and paper towels.
QUATS sanitizer measured at 200 ppm.
The inspection was conducted at the end of their operation where many of staffs started to leave. The inspector was not able to measure the high temperature dishwasher at this time.
