Observation: Mouse droppings observed in walk in cooler again along floor-wall joint on both sides of the main door as well as along the wall with glass doors (under and behind shelving where dairy is stored).
Corrective Action(s): Clean up mouse droppings and continue to check traps, resetting and rebaiting them as they are set off or bait is removed.

Observation: Holes/gaps observed in multiple locations throughout kitchen area that may be contributing to mouse activity include (but not limited to):
- bottom side of metal plate on wall near floor level behind prep/pizza area
- gap bottom left of walk in cooler door a tile is broken creating a gap which mice may be getting through
- in dry storage room: back lower shelf there is a gap with mouse droppings around it. *this was mentioned in previous inspections*
Corrective Action(s): Have all gaps sealed/filled with a material impervious to mice. Due: March 28, 2023

Comments

- Significant improvement in sanitation and in addressing many of the pest entry points.
- Metal flashing installed under line coolers in main grill/line area in kitchen; this will help with reducing the amount of food/debris getting under these units however they could also act as hiding places for mice. Strongly recommend sealing up remaining areas/gaps around units with a material impervious to pests.
- Inspection with pest control company done to help identify areas where mice are moving through premise and how to best approach addressing the persistent mouse issue. Staff stated they haven't received report from pest control company to date. Follow up with pest control company and see when the report can be expected. Forward a copy of the report to undersigned EHO when received.
