Observation: 1. 2 plastic covers with large cracks noted over food - nacho chips & cut veggies
2. Light fixture over back dry storage area
3. light bulb burned out / light cover missing over light in walk in freezer.
4. Light cover missing over light fixture over dishpit area
5. Temp gauges for both "alto-shaams" units.
Corrective Action(s): Ensure all fixtures/equipment are functioning properly.
1. Replace all broken equipment/containers - corrected during inspection
2./3./4. All light bulbs and fixture must be functioning properly and covers in place. Replace burned out bulbs, replace light covers and repair light fixtures. - Operator has electrician scheduled to come in
5. Temp gauge should be operating properly to indicate to staff that all hot held food is maintained at or above 60 C. - Operator is checking temps of food in unit 4 times a day and recording in log - satisfactory

Observation: Damaged metal corner wall protector by back door - partially ripped off wall and bent out. Bent protector is exposing unprotected wall surface.
Corrective Action(s): Ensure all surfaces in kitchen are smooth, durable, easy to clean and functioning properly. Repair/replace corner protector.

- Manual Dishwashing Method used for larger items. 200 ppm quats sanitizer used to sanitize equipment.
- Wiping cloths stored in 200 quats sanitizer available.
- Quats sanitizer in spray bottles also available
-General sanitation is satisfactory - major improvements noted

-No signs of pest activity noted during inspection
* Staff advised that Pest Control Company provides pest control service once a month.
