Observation (CORRECTED DURING INSPECTION): No liquid soap equipped for the hand sink at the kitchen. Hand washing without liquid soap can not be properly preformed, and may transmit pathogen to food.
Corrective Action(s): Refill the liquid soap into the soap dispenser

Observation (CORRECTED DURING INSPECTION): Kitchen staff were noted of wearing single use gloves, and preforming multiple tasks without changing the gloves or washing hands at the adequate frequency. Hand washing is important to prevent the transmission of foodborne illnesses.
Corrective Action(s): Kitchen staff must change their gloves or wash their hands at the adequate frequency.

Observation: Yearly permit fees, of $150, have not been paid. They were due in April 2022.
Corrective Action(s): Immediately pay the permit fee, and obtain a valid permit.

Observation: - Food debris and dust observed at various places (Floor, shelving, etc.)
- Blood stain and food debris observed in the freezers and coolers.
Corrective Action(s): Conduct a through cleaning of the kitchen floor and shelving. Clean the blood stain and food debris in the freezers and coolers. It is important to keep the premise clean to prevent pest harborage.

Observation: - Duct tape observed in the middle of prep sink and drain board, and the drain board and 3 compartment sinks.
- Plastic covering observed o the stainless steel shelving.
Corrective Action(s): - Remove the duct tape, and obtain the stainless steel covering to prevent water dripping to the floor. Duct tape is not a suitable material to be used as it can not be cleaned properly.
- Remove the plastic covering on the stainless steel shelving to allow the better cleaning of the shelving, and prevent pest harborage.

Observation (CORRECTED DURING INSPECTION): Single use gloves found to be used more than once, and not been change at the adequate frequency.
Corrective Action(s): Single use gloves should be changed more frequently while preforming different kitchen duties to prevent the transmission of pathogen.
