Observation: The dough mixer was very dirty and has build up of dried flour/dirt.
Corrective Action(s): The dough mixer should be cleaned every 4 hours if continously used, or clean it right after each use if just using once per day. Thoroughly clean and sanitize the dough mixer. Do not use the dough mixer until it has been thoroughly cleaned.

Observation (CORRECTED DURING INSPECTION): The sanitizer spray bottle had 0 ppm chlorine solution inside.
Corrective Action(s): Sanitizer spray bottles using chlorine for sanitization need to be at 100 to 200 ppm concentration to effectively destroy majority of the foodborne pathogens. The operator prepared a new solution of sanitizer and it measured to be 200 ppm chlorine solution.
