-Ownership change, DBA change + menu change
-Same type of cooking/process as previous restaurant
-Renovations completed in seating area. No changes to kitchen.
-Food safety procedures + sanitation procedures submitted. Copy of Operator's Foodsafe level 1 certificate submitted.
-Application form + certificate of incorporation submitted.

Approval to operate is not granted at this time. The following must be completed and verified with a follow up inspection:

1) Clean + sanitize all food contact surfaces in the kitchen. Residue noted on prep table, cutting board + meat slicer.

2) Clean dishwashing area backsplash to remove grime

3) Remove any equipment/supplies not required for the operation of the business including any equipment which is not in good repair (ie. wooden cutting board with deep groves/damaged, worn surface)

4) Organize kitchen so that equipment/supplies are in a designated space.
Organize dry storage room(s) to ensure chemicals are stored away from food products.

5) Supply/install paper towel dispenser in bar area adjacent to handsink.

6) Supply thermometers to all cold holding units.

7) Obtain probe thermometer to for measuring food temperatures

8) Obtain chlorine test strips for verifying chlorine residual from dishwasher.

Once all items are complete, contact the undersigned for a follow up inspection.
