Observation: 1. Gap noted under back exterior door to garbage area
2. Rat bait box noted inside building near back exterior door
Corrective Action(s): 1.Ensure all exterior doors have tight fitted door sweeps to prevent possible pest entry under door. Install new fitted door sweep.
2. Ensure all bait boxes remain outside facility - poison (bait) must remain outside. Only approved pest control devices inside facility is snap traps and sticky traps. Relocate bait box outside.

Corrective Action(s): Ensure chemicals are used for its intended purpose as per manufacturer's instructions.
1. Sani Derm is not a surface sanitizer it is intended to be used for hand sanitizer only - cease using for surfaces
2. Refill/ replace Spectrol or equivalent surface sanitizer / surface disinfectant product - use this product for disinfecting surfaces
