Observation: * Walk-in cooler was not working or being used again and not on; operator to maintain coolers at 4'C/40'F or colder including walk-in cooler too; operator turned on but it was 21'C-- it requires repair; glass over sushi cooler cracked and requires repair/replacement as not safe
Corrective Action(s): * Ensure walk-in cooler maintained at 4'C/40'F or colder at ALL times; keep this running as it is required for operation; ensure walk-in cooler working properly and repair; repair glas over sushi cooler or replace with one that is free of breakage
