209 - Food not protected from contamination [s. 12(a)]
Observation: 1. Sticky fly trap with dead flies hanging above the food hand-off area.
2. Ice machine visibly dirty.
3. Portable fan was being used to cool down hot foods.
Corrective Action(s): 1. Do not place fly traps directly above food as dead flies can fall on top of food. Please remove.
2. Ice machine must be cleaned and sanitized so that is visibly clean. Increase the frequency of cleaning ice machine.
3. Do not place fans directly above food as fans can be contaminated with dust and physically contaminate the food. Discussed ways to quickly cool down foods. Do not use fan to dry dishes dishes should be air dried.
Correct immediately.

304 - Premises not free of pests [s. 26(a)]
Observation: Rodent droppings observed in the corners of the dry storage area located adjacent to the exit door. Flies also observed in dry storage area.
Corrective Action(s): Remove all rodent droppings and clean and sanitize the area. Clean and sanitize dry storage area so that is no longer sticky and acting as an attractant for flies. Correct immedietely.
Note. Pest Control services are in place.

Observation: Walls and floors behind and underneath equipment and shelves visibly dirty.
Corrective Action(s): Ensure all vertical and horizontal surfaces are cleaned and sanitized so they are visibly clean. Continue to clean these surfaces on a routine basis. Correct by July 30, 2021.

Sushi bar cooler 4 C or below.
Beverage cooler around 6 C. No potentially hazardous foods stored in here.
Hot held foods greater than 60 C.
Sushi rice available. Operator stated acidified sushi rice is made as per food safety plan.
High temperature dishwasher registered at 75 C on plate surface.
***Note: Temperatures for coolers/freezers/hot holding and dishwasher not recorded. Ensure temperatures are checked and recorded daily.
Sanitizer solution available in kitchen and sushi bar - registered around 100 ppm for chlorine.
***Note: Wiping cloths should be stored in solution when not in use.
Hand washing sinks equipped with hot cold running water, liquid soap and paper towel.
