2-103.11(L)   Employees are not properly trained in food safety as it relates to their assigned duties, such as wearing gloves when touching ready to eat foods, and datemarking foods.
The Person in Charge or certified food manager shall train all his/her employees in food safety as it relates to their assigned duties.
46-36(a)  Corrected During Inspection Critical There is no Certified Food Manager present at the beginning of the inspection.
The certified food manager is responsible for being present during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment, utensils, and surfaces.
2-201.11(A)(1)-(5)  Corrected During Inspection Critical Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy."
A complete employee health policy must have the following elements: 1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout); and 2) Documentation that employees have been instructed of their responsibility to report symptoms of, diagnosis of or exposure to foodborne illness to management (see "Employee Illness Reporting Agreement" handout); and 3) A management plan to restrict or exclude employees, as applicable, who have symptoms, diagnosis or exposure to foodborne illness (see "Employee Illness Decision Guide for PIC/CFM" handout). The information provided at the time of this inspection will help you develop and implement this policy. 3-301.11(B)  Corrected During Inspection Critical A food employee was observed handling the following ready-to-eat food using their bare hands: bread
Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.
5-205.11(B)  Corrected During Inspection Observed the handsink at the front and back handsink being used to hold wiping cloths and utensils.
Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.
4-601.11(A)  Corrected During Inspection Critical The following equipment/utensils were observed soiled to sight and touch: slicer
Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
3-501.16(A)(2)(a)  Corrected During Inspection Critical The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cooked meatballs in the top portion of the Delfield two door prep cooler 45f, individually porportioned ranch dressing containers in the nonworking Edesa 3 door prep cooler 75f-78f (discarded)
Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
3-501.17  Corrected During Inspection Critical The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: Bulk opened deli turkey, Deli ham, cottage cheese in the walk in refrigerator
Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
3-501.17  Corrected During Inspection Critical The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: Lasagna and cooked lasagna noodles in the walk in refrigerator
Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees.
3-603.11(A)   Critical A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked: Hamburgers and steak
If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. As a temporary solution the 10 in house menus should be updated immediately and the carryout menus, once ready for reprinting or within 6 months, shall be printed with the appropriate consumer advisory. Sample menu's with the proper consumer advisory were provided during today's inspection.

Comments:


The purpose of this visit is a risk factor assessment. A certified food manager must be onsite during all operating hours. NOTE: The Edesa 3 door prep cooler across from the pizza oven which was being used to hold salad dressings and is not currently plugged in. The salad dressings were holding at 75f-78f. Do not place potentially hazardous food in this unit until it is plugged in and prechilled and capable of maintaining phf at 41F or below. A sign was place on this unit temporarily reminding employees not to use. Place individually packaged containers of phf salad dressing on ice. Thank you.
