304	Critical Inadequate Water Quality / Disinfection / Treatment / Quantity Observation: 25 Inadequate treatment for the removal of parasitic cysts of Giardia / Cryptosporidium Corrective Action: Well is shallow and under surface influence. No treatment in place for removal/inactivation of giardia and cryptosporidium.
304	Critical Inadequate Water Quality / Disinfection / Treatment / Quantity Observation: 17 Water sample positive for fecal coliform Corrective Action: Issues with operation of disinfection system resulted in over 45% of samples submitted in 2008 showing presence of e. coli and/or total coliform bacteria. a Boil Water Advisory was instituted and remains in place at this time.
310	Other (Specify) Observation: 05 Non-compliance with the conditions of the Operating Permit Corrective Action: The Construction Permit process has not been completed and a CP has not been issued yet for completed works.
