409	Total coliform exceedance Observation: 4 out of 10 samples collected since last inspection date were positive for total coliforms. This exceeds the maximum acceptable percentage of 10%. Corrective Action: Engineering study underway to determine most appropriate method of resolving ongoing Shelter Point area water quality issues. This may include consolidation of sources and elimination of some.
410	Critical Water sample positive for E. Coli Observation: Sample collected Feb 28/12 was positive for e. coli. A sample collected June 18/12 was also positive for e. coli. Corrective Action: A Boil Water Notice was placed on this water supply system upon notification of the first positive sample. Follow-up UVT testing results were very low, indicating UV treatment will not work effectively at all times on this water supply.
501	Critical Failure to disinfect surface water Observation: No chlorination in place for this GUDI water source Corrective Action: Engineering study underway to determine most appropriate method of resolving ongoing Shelter Point area water quality issues.
