404	Failure to monitor the water at the frequency required by the regulation, the operating permit or the directions of the DWO. Observation: Only 2 bacteriological samples submitted in 2017 to date. Corrective Action: Ensure monthly samples are submitted from this point on.

Comments:
Well is located inside a small building which is in need of replacement, as it is dilapidated and not vermin-proof . Well casing is only slightly elevated off the cement pad, therefore you should ensure steps are taken to allow any possible runoff water to drain away from the wellhead. Sampling frequency has not been adequate in 2017 to date; this will be rechecked at the end of the fiscal year (March 31, 2018).
