401	Critical Failure to provide potable water Observation: This water contains metals in levels exceeding the Maximum Acceptable Standard. Corrective Action: Submit a Construction Permit for a treatment system to remove/reduce the metals content in the water.
409	Total coliform exceedance Observation: Greater than 10% of samples submitted in 2011 were positive for total coliforms. Corrective Action: The treatment system to be proposed must ensure the bacteriological water quality is acceptable.
501	Critical Failure to disinfect surface water Observation: This is a surface water supply which must be disinfected. Corrective Action: The proposed treatment system must include disinfection.
