113   Improper Sanitary Facilities
Observation: Inadequate handwashing facilities – wall-mounted paper towel dispenser not available at 2compartment sink in food prep area (wall-mounted liquid soap available) in 10th Tee Shack.
Corrective Action: Handbasins must be kept clean and supplied with liquid soap and paper towels at all times during operation.Please provide paper towels for handwashing - must be wall mounted at 2compartment sink in 10th Tee Shack.
116   Improper Construction / Maintenance of Establishment
Observation: Floors and counters in 10th Tee Shack not properly constructed and installed, not in good repair, unclean surfaces.
Corrective Action: 1.The floors in the 10th Tee Shack must be thoroughly cleaned and kept clean -esp under shelves and in corners .2.Flooring in 10th Tee Shack must be replaced in 2005. 3.Counters in 10th Tee Shack must be replaced as soon as possible and sealed at counter/wall join to prevent the collection of dirt and debris in gaps.

Corrective Action: Display Supplementary Permit to Operate in view of public in 10th Tee Shack.

-wiping cloths kept in kept in sanitizing solution (200 ppm QAC) at all times in 10th Tee Shack and food prep area in SVR - spray bottles also available = good!.
