Observation: Flour beetles were noted along the baseboards where cornmeal bin is stored - they appeared to be Confused Flour Beetles.
Corrective Action: This facility has had problems with flour beetle infestation in the past. Begin by thoroughly cleaning all floors and along baseboards removing as much food debris and cornflour as possible. Seal all cracks and crevices with caulking to eliminate harborage areas. A vacuum may be useful. All dry foods should be sealed in pest proof containers with tight fitting lids. The cornflour bin should be emptied and disinfected regularly. Do not mix old cornflour with new cornflour. Thoroughly inspect all bags of dry foods prior to opening. This problem should also be discussed with the pest control company.

Observation: Overall maintenance of the establishment is deteriorating. A number of tiles are missing along the baseboards, drywall is chipped or cracked in several areas, many of the corner panels on walls are also loose which makes cleaning difficult as food is accumulating between panels and walls. Grease staining on ceiling and upper walls has also increased since last inspection. HVAC system is currently being worked on.
Corrective Action: Staff indicated that a renovation may be planned in the near future. This will be discussed with facility owner.

Observation: Some of the bottom shelves in the walk-in cooler are sitting directly on the floor preventing proper cleaning.
Corrective Action: All shelves must be at least 6 inches off the floor.

Comments:

Items Corrected from December 2010 Routine Inspection Report:
1. The hole in the wall under the hand sink has been covered with a plastic grate.

Outstanding Items:
No shelving has yet been provided for the back area where pop and pizza boxes are stored. There are too many stationary storage piles that cannot be easily moved for daily cleaning duties. In ability to properly clean along walls and remove food debris is likely exacerbating the flour beetle problem. All stored items must arranged in a way that provides a 6 inch clearance underneath to facilitate proper cleaning.

As per June 2, 2010 and December 8, 2010 reports, documentation from the Domino's Food Safety Plan must be provided that confirms room temperature storage for pizza sauce.

General Observations:
1. All temperatures met regulatory requirements.
2. Thermometers noted in coolers.
3. Temperatures are recorded twice per day.
4. Hand sink supplied with liquid soap and paper towel.
5. Foodsafe Level 1 Certified person on site.
6. Food contact surface sanitizing solution tested 200ppm quat - good.
7. Food storage acceptable - foods are covered and dated.
