Observation: 1) Tap faucet is leaking at handwash sink in bar.
Corrective Action: 1) Install new tap/faucet at handwash sink. Repair all leaks.

Observation: 2) Handwash sink counter area is worn and damaged.
Corrective Action: 2) Replace counter at handwash sink area. Provide a countertop that is smooth, durable, easy-to-clean and free from cracks and crevices.

Observation: 3) Ice machine room requires cleaning and reorganizing. Ice machine plumbing connections have not been inspected or approved by DOS Building inspector. Ice machine is currently not in use.
Corrective Action: 3) Ice machine room requires the following: Finish all floors, walls and ceiling surfaces to provide surfaces that are smooth, durable and easy to clean; provide metal rack shelving for storage of packages of plastic ice bags (to be stored inside storage bins); provide food-grade storage bins (white or clear plastic) with tight-fitting lids to store plastic ice bags inside, so ice bags are maintained in a clean and sanitary manner. Ice machine cannot be used until written approval from EHO is given. Ice machine must have all plumbing connections approved by DOS Building Inspector. Inside of ice machine must be emptied of ice, cleaned and sanitized prior to use.

- Glasswasher provides 100ppm chlorine in sanitizing rinse.
- Liquid handsoap and paper towels provided at handwash sink.
- Sanitation log maintained daily.
