Comments:

Facility was reinspected to assess lifting the closure order issued on September 22nd, 2010.
The following observations were made:
1. Pest control company had attended the site at 7:30am on the day of inspection to place more glue traps in the downstairs area as well as check existing traps for signs of activity. A report of the findings was submitted to the health department in which pest control operator advised "upstairs of the building is safe for food service."
2. Double door frame downstairs has been sealed with steel wool and foam insulation. These doors will be kept closed at all times and designate the separation between food/restaurant equipment storage and the rest of the facility.
3. Main area downstairs has been cleared of all clutter and unnecessary supplies. All adjoining rooms have been cleaned and sanitized. Floors and walls have been cleaned.
4. Door sweeps have been installed on all downstairs doors and works are underway to completely seal off all potential access points using foam/steel wool and/or drywall.
5. All banquet dishware and glassware are now stored in designated pest proof containers.
6. Discussed the feasibility of the back storage room (currently used for pop) to be converted to a dry storage area including the installation of shelves and additional lighting.
7. Discussed ongoing pest management program including: weekly visits by pest control company until the problem is completely under control and continuing construction work to completely seal off all potential access points.
8. Dishwasher technician has planned a second maintenance visit to the facility for next week to repair the glass washer. Until this time, all bar glassware will be washed using the high temp dishwasher in the kitchen.

Facility has been granted permission to re-open subject to the following conditions:
1. All items contained within the letter dated September 24th are completed according to scheduled dates.
2. All items contained within the inspection report dated September 22nd are completed as per dates discussed.
