-Sanitation Plan (cleaning schedule for equipment and chemicals used)
The following items must be completed prior to operating:
-Soap and paper towel dispensers mounted at hand wash stations
-Thermometers in all coolers
-Sanitizer dispensing station installed, sanitizer available for all food contact surfaces
-Temperature logs for all coolers in place
-Remove all construction materials
-Thoroughly clean and sanitize all surfaces
-Sneeze guards in place
Permit to operate issued effective today.
Permit Conditions:

-Alterations to menu or food services must be pre-approved by the health department.
