112	Poor Sanitary Design, Construction, Installation of Equipment & Utensils Observation: The pump for the detergent for the glasswasher requires repairs. Corrective Action: Ensure that the pumps are repaired.
113	Improper Sanitary Facilities Observation: Two female toilets are broken. Corrective Action: These must now be repaired by the follow-up inspection date.
113	Improper Sanitary Facilities Observation: The door on one of the female washrooms is broken. Corrective Action: This must now be repaired by the follow-up inspection date.
113	Improper Sanitary Facilities Observation: One male bathroom stall toilet is not working. Corrective Action: This must now be repaired by the follow-up inspection date.
115	Inadequate Insect / Rodent Control Observation: Some fruit flies noted. Corrective Action: Provide plugs for the floor drains. Implement a regimen of cleaning the drains with an appropriate drain treatment for fruit flies (such as a concentrated quaternary ammonium cleaner or a foaming enzymatic cleaner). This should be done at least weekly. Also, provide plugs for all sinks in the bar area and seal up all of the drains at the end of each night.

Complaint inspection also conducted this date regarding alleged unhygienic staff behavior. Complaint was discussed with management.

Routine inspection conducted this date. Glass washer dispenses 12.5ppm iodine as required.
