Observation (CORRECTED DURING INSPECTION): - Bus pan of potatoes stored in cooler uncovered with tray stored on top. Bottom of tray in direct contact with potatoes. This is unacceptable and unsanitary. Bottom of tray is not clean, sanitized.
- Numerous containers of food stored in cooler uncovered.
Corrective Action(s): - Items were covered when Operator was notified. Ensure all items in storage (dry or refrigerated) are covered tightly to protect the food from contamination.

304 - Premises not free of pests [s. 26(a)]
Observation: - Mouse droppings noted in the following areas:
- on floor under table in hallway
- on floor under shelving in dry storage room
- on stainless platform where dishwasher is located.
Corrective Action(s): - Clean up droppings and disinfect areas where droppings noted.
- Continue to monitor for any new activity, cleaning up droppings immediately if noted. If rodent activity continues, a pest control contract will be required.

Observation: - Food debris noted under shelving in dry storage room. Room is cluttered.
- Grime and moisture noted under 2 compartment dishwashing sink
- Grease accumulation and drippings noted on wall adjacent to oven
- Food debris noted inside sliding door coolers.
Corrective Action(s): - Remove all moveable items from the floor and thoroughly clean entire floor to remove all grime, moisture and food debris. Keep floor area clear to facilitate cleaning and monitoring for pest activity.
- Clean the interior of all sliding door cooler units.
- Thoroughly clean wall to remove grease drippings.

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation: - High temperature dishwasher was not operational. Operator reported this at the beginning of the inspection and that a new dishwasher was scheduled to be installed today. Premises was closing for the day and dishes were being rinsed and will be washed and sanitized when new dishwasher is installed and operational.
Corrective Action(s): - Repair/replace dishwasher and wash and sanitize all dishes prior to re-opening for regular service. - Technician arrived on-site at time of inspection to install new unit. Provide confirmation to district EHO once complete.

- Bleach surface sanitizer solution present in kitchen and front service area. Concentration measured 200ppm chlorine in front service area and >>200ppm chlorine in kitchen. Dilute to 100-200ppm chlorine.
- Hot holding of potentially hazardous food was at acceptable temperatures (60C or above)
- Soup 70-78C
- Cooked rice 67C
- Observed soup cooling in cold running water bath. Reviewed cooling procedures with Operator.
- Exhaust canopy is due for service March 2020. Ensure filters are cleaned more frequently to prevent grease accumulation.
- Operator holds valid Foodsafe level 1 certificate.


*Propane burners to be removed as per Fire Dept. Completed at time of inspection.

* Direct food handling or food preparation should not occur in back hallway as there is no handsink present and surfaces are not smooth and washable. Approved plans to be reviewed to confirm original layout which was approved.
