Observation (CORRECTED DURING INSPECTION): Observed food (lettuce and tomatoes) in the sanitizing sink where clean dishware are being sanitized.
No sanitzer available in bucket or spray bottle at all. Only sanitizer is found in second sink.
Corrective Action(s): Properly wash and RINSE off food in the same sink and ensure that all dishware and equipment are properly cleaned before submerging clean items into second sink for sanitizing. New staff was instructed to resanitize all dishware, emptied second sink and restart with 200ppm Quats.
Instructed staff to make 200ppm Quats for food contact surfaces for wiping towels.

Observation: Mice droppings identified in several areas: cupboard where taco shells are stored (1 dead mouse caught), behind two compartment sinks, beside drying rack and back storage shelving units all along floor.
Corrective Action(s): Please remove droppings and carcass. Pest activitiy needs to be monitored more frequently through dropping removal and increase sanitation. Provide pest control reports from last three months to date for review. Email or provide onsite at next visit.

Observation: Food debris noted under hot holding unit in the front service area and under dry storage shelving units.
Corrective Action(s): Please remove, sanitation level needs to improve especially when there is pest issue.
