Observation: 1. Walk in freezer has been converted to a walk in cooler with out approval
2. Multiple pieces of refrigeration have been moved around and are in new locations and large rotisserie was added with out approval
3. Ceiling tiles and light covers are missing some since last report.
4. Multiple holes in the walls noted, which need to be filled (i.e. under dishwasher)
5. Dirty side of dishwashing area and hand sink by ice machine are no longer properly secured to the wall and can allow for pest harborage.
6. Multiple tiles are missing or broken on coving through out premises
Corrective Action(s): 1. Submit an up to date drawing of proposed floor plan for kitchen (including location of all equipment, coolers, and freezers) for review.
2. Replace all missing ceiling tiles and light covers
3. Repair all holes in wall and all missing/broken tiles
4. Repair seals between metal sinks/drain boards and walls to prevent pest from hiding in gaps between the 2.
