Observation (CORRECTED DURING INSPECTION): Found chicken, tomato sauce, pepperoni all sitting between 6.6 -8.7 C for more then 2 hours, cooler does not have proper lid.
Corrective Action(s): Discard items immediately. Use only small portions that can be used up in 2 hours, discard after until proper lid is purchased or new unit is installed.

Observation (CORRECTED DURING INSPECTION): Noted one hand sink blocked by nacho chips, and the other sink had no paper towel that was easily accessible.
Corrective Action(s): Ensure all hand sinks are easily accessible and well stocked with liquid soap and paper towel.

Observation: Found heavy fruit fly infestation up at bar, inside container on fruit (limes and lemons).
Corrective Action(s): Have pest control assist with problem, as current in house made traps are not working.

Observation: Heavy food build up noted under, in between and behind all equipment and shelving along cook and prep lines.

Observation: 1. Noted a few cutting boards that were heavily worn and can no longer be easily washed and sanitized.
2. Slim upright freezer has light hanging exposing fiber glass and wires
3. meat slicer had cut wires on electrical cord.
Corrective Action(s): 1. Discard all worn cutting boards and replace with new easily cleanable ones
2. Repair broken light in freezer.
3. Repair cut electrical cord or replace meat slicer entirely.

Observation: Found multiple containers for dairy products being reused to store alternative foods such as cereals.
Corrective Action(s): Must only use reusable food grade containers to store food. Do not reuse containers from commercially prepared items after food has been used. Change all containers over to reusable containers.

311 - Premises not maintained as per approved plans [s. 6(1)(b)]
Observation: 1. Walk in freezer has been converted to a walk in cooler with out approval
2. Multiple pieces of refrigeration have been moved around and are in new locations and large rotisserie was added with out approval
3. Ceiling tiles and light covers are missing some since last report.
4. Multiple holes in the walls noted, which need to be filled (i.e. under dishwasher)
5. Dirty side of dishwashing area and hand sink by ice machine are no longer properly secured to the wall and can allow for pest harborage.
6. Multiple tiles are missing or broken on coving through out premises
Corrective Action(s): 1. Submit an up to date drawing of proposed floor plan for kitchen (including location of all equipment, coolers, and freezers) for review.
2. Replace all missing ceiling tiles and light covers
3. Repair all holes in wall and all missing/broken tiles
4. Repair seals between metal sinks/drain boards and walls to prevent pest from hiding in gaps between the 2.

Comments

Walk in cooler which was a freezer was 4 C
- Walk in cooler was 2 C
- All other coolers except top portion of pizza cooler were 4 C or colder
- 3 door freezer was - 12 C
- Slim freezer was - 10 C
- Hand sinks had hot and cold water, liquid soap, and paper towel
- Spray bottles had ~ 200 ppm Quat, ensure to monitor concentration daily
- Dishwasher had a final rinse of 78 C at plate and 82 C at gauge
- Glass washer had a final rinse of 100 ppm chlorine
- Pest control is on contract, unable to verify reports as not on site. Have reports submitted for review.

NOTE: if wish to add rotisserie chicken to menu, submit an up to date food safety plan which reflects the added menu item.
