Operator informed that the mesh is broken to put a chain through the door for a lock. Corrective Action(s): Place a new mesh on screen door and ensure screen door is properly secured to prevent pest entry. If you want the back door open, a proper screen door/net with secure mesh is required. To be corrected by August 1, 2017. Violation Score: 9
