Observation (CORRECTED DURING INSPECTION): Observed staff rinsing hands with gloves still on, dried hands and about to make tempura.
Corrective Action(s): Instructed staff to remove gloves, wash hands and wear new pair of gloves. Do not wash hands with gloves on, this is improper hand hygiene.

Observation: Food debris and grease collected in many corners of kitchen, under and between fridges, prep tables and shelving units. Photos taken. Repeat.
Corrective Action(s): Deep clean needs to happen weekly. Declutter hallway and dry storage room. Correction order issued to maintaining sanitation.

Observation: Cardboards being used to line flooring throughout kitchen. Repeat.
Corrective Action(s): Do not line floor with cardboard as it is not a suitable cleaning material. Remove forewith.

Observation: Dishwasher has been leaking since last week as informed by staff. Two bins are placed underneath dishwasher unit and cardboard placed on floor to soak up water as means to fix problem. Faucet at two compartment sinks is loose.
Corrective Action(s): Operator informed he will call for repair at time of inspection. This is unacceptable. Repair leak immediately. Secure faucet at two compartment sinks.
