A follow-up inspection was conducted in regards to the violations observed on February 28, 2019:
Light bulbs have been replaced. Kitchen is adequately lit.
Cleaning has been completed
Dishwasher motor was replaced. Dishwasher reached >71C at the plate level upon final rinse cycle (min/max probe thermometer).
