Observation (CORRECTED DURING INSPECTION): The following foods/ packages of food had rodent chew marks on them:

Storage room:

-One 20 kg bag of Calrose Shirakaku Rice
-One 3.3 lb bag of karage mix
-One squash stored in a box

Kitchen:

-One 1kg bag of beef soup stock
-One bag of sugar (labelled in another language)

301 - Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
Observation: There is no food contact surface sanitizer available for use. There was only one bleach bottle that had 0 ppm bleach and was broken.
Corrective Action(s): Prepare several 100 ppm bleach solutions for the re-opening inspection.

Observation (CORRECTED DURING INSPECTION): Items run through the dishwasher were not being properly sanitized. See code 308 for details.

Observation (CORRECTED DURING INSPECTION): Repeat Violation: The back hand washing sink was obstructed. There were trays stored on top of it and containers stored in front.
Corrective Action(s): Corrected during the inspection.

304 - Premises not free of pests [s. 26(a)]
Observation: Severe pest infestation observed at the time of the inspection. Rat droppings observed in the following areas:

-On the floor underneath the back hand washing sink and the adjacent shelving.
-On the floor and on the shelving in the back storage room.
-Inside a box of paper towels in the storage room.
-Underneath the front prep coolers and underneath the kitchen 2-door cooler.
-On food containers in the kitchen

Rodent chew marks observed on several food items, as noted in violation code 201.
Corrective Action(s): The restaurant has a pest control company, and the report was reviewed at the time of the inspection. However, sanitation is poor and pest proofing is needed. Have a pest control company come onsite again and fully pest proof the facility which includes checking for rodent entry points and seal them.

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: 1) Gap noted underneath the kitchen door. This can allow rodents to enter the facility.
2) The back storage room has an open window.
3) Many foods were not stored in pest proof containers.
Corrective Action(s): 1) Check all doors and ensure that they have proper fitted door sweeps.
2) Keep the back storage room window closed or install a proper screen there.
3) Purchase pest proof containers with fitted lids for all foods

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Sanitation was poor. Grease, food debris buildup observed throughout the restaurant and storage areas.

Cleaning required in the following areas:

-Underneath all equipment. The floor underneath the prep coolers in the front needs extensive cleaning to remove food debris, droppings and grease buildup.
-Along walls.
-On shelving
-In the food storage room
-All flooring
Corrective Action(s): Correct prior to re-opening.

307 - Equipment/utensils/food contact surfaces are not of suitable design/material [s. 16; s. 19]
Observation: One of the chest freezers in the back storage room has a broken door and insulation is visible.
Corrective Action(s): Repair or replace this freezer prior to re-opening.

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation: The high temperature dishwasher did not achieve greater than 71 C at the plate level after 5 runs. It was turned on several hours earlier. The maximum temperature achieved was 67.3 degrees Celsius.
Corrective Action(s): Repair the dishwasher so that it can consistently achieve a plate temperature of over 71 C before re-opening

Comments

Closure Order issued due to a malfunctioning dishwasher, poor sanitation and a rodent infestation.

A re-inspection is required once all of the above noted items are corrected.

Note: The outside garbage and organic bins were overflowing and styrofoam fish containers were stored outside. This can attract rodents and should be corrected.

Inspection notes:

All coolers were < 4 C.
All freezers were < -18 C.
Hot held foods were > 60 C.
Hand washing sinks were supplied with hot and cold water, soap and paper towels.
