Observation: Both the detergent and sanitizer dispensers for the dishwasher had ran dry. As a result, no sanitizer was detected.
Corrective Action(s): Bleach had been added to the sanitizer dispenser. Detergent had been replenished. Check chemical levels daily.

Observation: Floor is in unacceptable condition. The vinyl tiles are chipped, broken and worn through in large patches with the subfloor exposed. The floor cannot be cleaned. Grime is stuck onto the porous subfloor.
Corrective Action(s): Replace the floor covering.

Sink waste water is leaking from the pipe under sink in the staff washroom. Repair the leak.
Keep nails short and trimmed to protect food from contamination.
