The following corrective actions have been taken: - Domestic dishwasher has been removed from premises. Food/beverage served to customers is with single service disposable utensils/dishes in accordance with condition on permit - Walk-in cooler has been re-organized to ensure all food items are stored off of the floor. Crate placed under box of potatoes so that they are not stored directly on the floor. For any future proposed alterations including changes to layout or equipment, ensure proposed plans and specifications are submitted to the EHO for prior review and approval is received prior to alterations being made. *Decal for health operating permit is currently affixed to City of Burnaby business licence. Health operating permit with valid decal is to be displayed in a conspicuous (visible) location. Operator was unable to locate health operating permit. Form for replacement permit provided to be completed and submitted if permit cannot be located.
