Observation: Back door not closing property; build up of debris and accumulation of unused items in the restaurant.
Corrective Action(s): 1. Ensure back door is kept closed at all times to prevent entry of pests.
2. Absolutely no dishes, utensils, pans etc are to be stored on the floor. All other items must be stored on shelves at least
6 inches off the floor.
3. Remove any un-used and unnecessary items and reduce clutter to prevent pest harborage.

Observation: Build up of flour, food debris and grease noticed on the floor throughout the kitchen as well as food storage shelves and counters.
Corrective Action(s): Thorough cleaning required to remove all debris build up (By January 7, 2018).

Observation: Plumbing leak noticed underneath the double sink; broken spray head on the wash sink.
Corrective Action(s): Repair the plumbing leak and broken spray head immediately.

* Install washable and non-absorbent sheets on the backsplash around the mop sink as well as dish washing sink. (By January 20, 2018).
* Discard any tea towels used in the premises.
* Recommended deep cleaning of the restaurant at least one a week.
