Observation (CORRECTED DURING INSPECTION): The inside cover for the freezer lid found to be broken exposing the food items to insulation materials.
Corrective Action(s): Secure the inside plastic cover with strong tapes immediately. Cover all food products stored in the freezer. Replace freezer if unable to find a replacement lid (Dec. 15, 2016)

Observation: Food build up noticed on the walk-in cooler shelves.Floor underneath the double sink require cleaning.
Corrective Action(s): Thoroughly clean all shelving in the walk in cooler. Remove fan covers to clean them thoroughly.
Clean area undernealth and on both sides of the double sink(By Dec. 14, 2016).

Observation: Build up of grease noticed inside the fume hood. No grease filters installed.
Corrective Action(s): Install grease filters on the fume hood. Fume hood must be serviced and cleaned by licensed technicians by Dec. 15, 2016.
A sticker must be affixed confirming date of service.
