Observation (CORRECTED DURING INSPECTION): Two bags of hot dog buns contaminated. Items had been eaten through bag from rodents - CORRECTED DURING INSPECTION.

Corrective Action(s): Pull all food and single use disposable containers out of cupboards and inspect. Any items that have been contaminated by rodents (through either chewing or droppings) dispose of.

Observation: There is currently an infestation of the following at the facility:

Corrective Action(s): The following actions must occur:
1) Have a pest technician service the facility IMMEDIATELY - replace traps, check for holes, make recommendations
2) Clean and sanitize all floors, inside cupboards, shelves, etc.
3) Seal all holes
4) Follow any suggestions made by pest technician
5) Clean and organize facility - remove any items not needed, make floor easier to clean and view for signs of infestations
6) Discard any contaminated food items
7) Move all items in storage locker up off floor

Observation: Cleaning needs to occur throughout. Current level of cleanliness on floor and in cupboards needs to improve. Rodent droppings found inside cupboards need to be cleaned and sanitized with sanitizer solution (200ppm QUAT)

COMMENTS:
- Freezer temperatures less than or equal to -18 degrees
- Cooler temperatures less than or equal to 4 degrees Celsius
- Dishwasher achieving final sanitizing rinse of greater than 72 degrees Celsius at the dish (85.5 degrees Celsius)
- Sanitizer bucket in the back at concentrations of greater than or equal to 200ppm QUAT

NOTE: Repeat violations from August 2015 inspections - violations observed: lacking soap from front handsink, preparation of orange juice in back hand sink, storage of buns on floor in back storage area, QUAT sanitizer solution at concentrations of less than 200ppm. These items have been brought to the attention of the facility on previous inspections. DO NOT REPEAT THESE VIOLATIONS.
