Observation: -Permit to operate not posted. This is a repeat issue which was identified on the previous routine inspection reports and needs to be addressed.
Corrective Action(s): -Post permit to operate with valid decal affixed in a visible location. Permit replacement form has been previously supplied if permit requires replacement. [Correction date: 1 month]

Observation: -Front display cooler was at 8C. Mostly baked goods and non perishables stored in the unit with the exception of whipped cream
Corrective Action(s): -Adjust/repair display cooler and do not store any potentially hazardous foods in unit until it can maitain 4C or colder.

-Premises and equipment maintained in sanitary condition with the exception of the floor under the deep fryers (noted in violation above)
-All handwashing stations supplied with hot and cold running water, liquid soap and paper towel with the exception of the bar handsink where the soap was inaccessible (noted in violation above)

-Hot holding temperatures of food in steam table and pizza warmer were acceptable (>60C). In use utensils stored in hot water (>60C) in steam table
-Dishwasher operational and capable of sanitizing dishes. 100ppm chlorine detected on rinse after cycle.
-Wiping cloths stored in chlorine sanitizer solution (100-200ppm)
-No signs of pest activity noted at the time of the inspection. Adhesive traps placed for monitoring. Pest control contract in place.
