Mildew from ice machine has been removed. Products have been organized and are stored 6 inches off the floor. Noted that new products were being delivered at time of inspection. Mice droppings from front service area, back area by walk-in freezer and dry storage space/staff locker space have been removed. ->droppings and new syrup spillage behind dry storage shelves (beside ice machine) still needs to be done; discussed with staff onsite, remind all staffs not to jam the products too hard as the syrup bottles in the back gets broken and therefore causes spillage ***also noted a live mouse that came out under the three compartment sinks at time of inspection (unable to determine if the mouse had gone underneath the grease trap or not); please have Orkin service this particular area and seek for entry point Owner has requested for permit to be replaced and shall be contacting billing department directly as per e-mail.
