Observation (CORRECTED DURING INSPECTION): * Upon arrival to inspection, sanitizer was not set up or readily available to sanitize food contact surfaces. Operator told to set up sanitizer and they made up bleach solution in bucket to store all wiping rags used to sanitize food contact surfaces. Operator has been told many times to have sanitizer readily available at ALL times while in operation. Again this is noted as a violation!
Corrective Action(s): * Ensure sanitizer readily available at ALL times while in operation and use it to sanitize food contact surfaces through out operational hours. Store ALL wiping rags in sanitizer between use.

Observation: * Pizza pan table being used replaced the old plywood table from last inspection, however, electricians tape used to hold edges together is considered unsanitary and requires removal or table replacement again. All food tables MUST be food grade and easy to clean/sanitize. Also, prepped onions/green peppers were being stored in card board boxes and this is unsanitary as these food were already washed and cut. Washed and cut vegetables, etc., must be stored in washable/sanitizable containers that are food grade type. Operator rewashed these prepped vegetables and stored in proper food grade containers.
Corrective Action(s): * Ensure ALL food equipment/tables in food grade condition and easy to clean/sanitize. Also, after washing and cutting vegetables, etc., ensure these foods stored in washable/sanitizable containers that are food grade containers NOT card board boxes!

Observation: * Operator was not wearing proper attire at beginning of inspection but then put an apron on once they realized inspection occurring. Operator reminded to wear proper attire while handling food(eg. apron/uniform)
Corrective Action(s): * Ensure ALL food handling staff wear proper attire and hair protection while preparing/handling food to reduce risk of contamination

* Update cleaning schedule monitoring as the records were not available; temperature monitoring records up to date
* Hand washing station had liquid soap and paper towels but soap level was very low; ensure soap dispensers have adequate amounts of soap available for hand washing
* Cold-holding units were 4'C/40'F or colder
* Hot-holding pizza unit was 60'C/140'F or hotter
* Sanitizer (bleach solution) was set up once operator was told to do so; sanitizer to be readily available at ALL times during operation and operator has been reminded many times during past inspections
* Freezer unit was -18'C or colder
* Note: Pizza pan table still not food grade as it was being taped together and this is unsanitary!
* Note: Ensure food that is prepped(ie. washed/cut) is being stored in food grade containers NOT card board
* Note: Staff to wear proper attire while handling/prepping food (eg. apron/uniform)
