Observation (CORRECTED DURING INSPECTION): Cooked breaded prawns observed stored under heat lamp in front kitchen area at 49.9 C.
Corrective Action(s): Breaded prawns were removed and reheated. Ensure all food is stored at 60 C or above to prevent the growth of bacteria and their toxins. Adjust the temperature of the heat lamp to ensure food is kept at 60 C or above. Time stamp any food stored below 60 C for a maximum of 2 hours then discard. A method of time tracking must be available if food is held between 4 C - 60 C.

Observation: Brown organic debris observed on the interior of ice machine.
Corrective Action(s): Discard all ice from the machine. Clean and sanitize the interior of the machine to remove all organic debris to prevent cross contamination. Ensure the ice machine is cleaned regularly. Date to be completed: immediately.

Observation: 1. Wall mounted electric fly trap observed placed on the counter by rice cookers. Several dead flies observed in interior of electric fly trap.
2. Raw shelled eggs stored above ready to eat vegetables in the walk-in cooler.
3. Various foods observed stored directly on the floor in the storage area, walk-in cooler, and walk-in freezer.
Corrective Action(s): To prevent the cross contamination of food or food contact surfaces:
1. Remove the fly trap from food storage/preparation areas. Contact your pest provider to install the fly trap on the wall away from all food and food contact surfaces.
2. Relocate the raw eggs away from all ready-to-eat foods. All raw foods are to be stored below ready-to-eat foods.
3. All food is to be stored at least 6 inches off the floor.

Observation: Several rodent droppings observed behind the chest freezer in the storage area, and under shelving in the front sushi area.
Corrective Action(s): Remove all rodent droppings wearing gloves and face mask. Disinfect the area with 200 ppm of chlorine bleach. Contact your pest control provider. Locate and seal any holes and gaps in the premises to prevent the entry of pests.

2. Long standing debris observed on the floor against the wall behind rice cookers in kitchen area.

4. Long-standing food debris observed on stove around the dials in kitchen area.
5. Reach-in cooler gaskets and seals in kitchen area and sushi area observed with build-up of food debris.
Corrective Action(s): Clean all areas indicated above to maintain the premises in sanitary condition and to prevent the attraction of pests.
