Observation: The handwashing station at the back did not have paper towels in dispenser. Paper towels provided above the dispenser (too high to reach). No other areas surrounding the sink was appropriate to place the paper towels.
Corrective Action(s): Provide paper towels in dispenser.

Observation (CORRECTED DURING INSPECTION): Multiple food items in containers not properly covered with lids in undercounter cooler and walk-in cooler.

Observation: Rodent droppings noted on floor near the water boiler and under the microwave shelf.

Observation: Operator's Food Safe Certificate was expired as it was taken along with other staffs, who's certificates were provided, but expired (taken in 2002 and 2004 and is expired on July 29, 2018 as of the new policy).
Corrective Action(s): Operator to retake the Food Safe Level 1 course or provide equivalent certification. In the absence of operator, at least one staff must be have valid Food Safe Certificate.
Correction date: 1 month.

-Chlorine sanitizer in spray bottle at 100ppm chlorine
-Operator discussed cleaning procedures for cutting boards and band saw. Parts are disassembled every 4 hours to wash, rinse sanitize with 100ppm chlorine bleach.
-Test strips used were pH test strips and were used to test chlorine concentration. Mentioned that proper test strips for chlorine should be provided.
-Discussed that the dough mixer should be cleaned at 4-hour intervals if not continuously used.
-Staff bathroom appeared cleaned, hand sink had hot and cold running water, paper towels and hand soap.
-Pest control report provided May 22, 2019 (one mouse caught) with contract in place. Operator mentioned he sometimes ask for more frequent services if necessary.
