Observation (CORRECTED DURING INSPECTION): Multiple Violations:
1) Sanitizer solution throughout less than 200ppm QUAT - CORRECTED DURING INSPECTION - Solution changed during inspection
2) Sanitizer solution from dispensing units less than 200ppm QUAT - CORRECTED DURING INSPECTION - Solution was made and tested to be at 200 ppm QUAT using manual pour.
3) Blenders are "rinsed in back" after each use. No sanitizing step used.

Corrective Action(s): Corrective Actions:
1) Ensure sanitizer solution is changed throughout the day at a high frequency
2) Sanitizer solution is to be tested daily. Currently staff are not testing the solution from dispensing unit. The testing solution has faded from light exposure indicating testing has not occurred in some time.
3) Ensure blenders are properly washed-rinsed-sanitized-air dried every two hours or more as necessary.

Observation: Rodent droppings found in bottom back cupboard.

Corrective Action(s): Spray rodent droppings with 200ppm chlorine solution or equivalent. Remove droppings from area. Wet affected area with 200ppm chlorine or equivalent and soak area.
Inspect facility for points of entry for pests. Seal any holes.

Observation: 1) Sanitizing dispensing unit is malfunctioning. The Sanitizer dispensed from unit is dispensed at concentrations less than 200ppm QUAT.
2) Door of front cupboard has come off hinges.

Corrective Action(s): 1) Manually dispense QUAT solution and test with testing papers to ensure concentrations of 200 ppm QUAT are reached.Have unit serviced by technician.
2) Ensure hinges are replaced.
