NOTE: operator voluntarily closed the restaurant on last Wednesday and Thursday to facilitate correction of the citations -No goods directly on the ground in hallway leading to back door -either on the roller carts or on a shelf -External storage room -risers have been removed -multi-level shelf with wheels available now and is movable to ease cleaning and monitoring of pests -No goods on the floor in narrow hallway. No goods on the floor area of the mop sink. -Personal effects only in office room or mop sink area. -No mice caught in the last pest control visit on November 22 -Orkin Pest Control to come in bi-weekly instead of monthly for the next few months -operator to email these reports to me NOTE: all corrections indicated by the pest control MUST be corrected
