Observation: 1. lower torso of roach observed in naan dough. Affected product discarded when identified.

Observation: 1. Walk-in freezer observed to have heavy build up of ice. Operator has contracted refrigeration technician several times without success.
2. Kitchen surfaces observed to be damaged (floor tiles, walls near dishwashing area and food preparation areas.)
3. Dishwasher not primed at start of inspection.
Corrective Action(s): 1. Clean area regularly. Do not store food directly on floor or in drop 'range' of ice.
