Observation: High temp dishwasher run multiple times - temperature at the plate noted with min/max probe thermometer by both EHO and Exec Chef between 65-70 C. 71 C reached once.
Corrective Action(s): ***REPEAT VIOLATION***

-Dishwasher is not reaching 71 C or higher consistently to sanitize dishes.
-CORRECTION ORDER remains in place
-Service has been called

***In the interim to remain OPEN, dishes must be manually sanitized in large pot sink after running dishes through the dishwasher. If dishes are not properly sanitized after washing the premises will be closed until such time that the high temperature dishwasher is functioning properly again.
***A dishwasher log must be developed where the dishwasher will be verified it is reaching 71 C or higher daily and if not it must be serviced immediately. Records must be available onsite for review by FHA upon request next inspection

***Contact FHA once dishwasher has been repaired

Observation: 1. High temperature dishwasher noted 65-70 C - temperature varies and is not consistent.
2. Prep table cutting boards appear deeply gouged and black color
Corrective Action(s): Ensure all equipment is functioning correctly and is in good working order

1. Repair dishwasher and follow CORRECTION ORDER issued Jan 20, 2020. If this Order is not followed CLOSURE may be required.

2. Replace/re-surface cutting boards to maintain them in good working condition and allow for easy cleaning and sanitizing.

Corrective Action(s): Ensure all chemical bottles/containers are properly labelled with its contents. Recommend getting workplace labels (stickers) from chemical suppliers to place directly on bottles/containers. This will meet FHA requirements and WorkSafe requirements. Temporary labels are acceptable in the interim.

Follow-up inspection to report #ZDER-BL23CB
-Screen door closed during inspection and the door has been repaired at the bottom and a temporary bungee has been added to help automatically close the door to prevent pest entry
*Operator has ordered a complete new screen door
*Pest control on-site during inspection - follow all pest control recommendations about securing all external door with door sweeps and filling in gaps to prevent other possible entry points. No major pest control issues noted during inspection
-Operator has discussed with cleaning company about cleaning hard to reach areas of kitchen including under prep table across from walk in cooler
-Tiles have been replaced in front of deep frier unit
-Light bulbs have been replaced above steamer unit and downstairs storage room
-Cooler drawer and cooler door have been repaired and duct tape removed
*Ice cream freezer lid has not been replaced yet - operator looking to replace unit completely with a small chest freezer
-Cooler under flat grill unit noted: 4 C
-Access panel with handle installed above mixer unit and gap in ceiling filled. Old broken panel has been removed.
