Observation (CORRECTED DURING INSPECTION): Yams stored in cardboard box with rodent droppings in box - CORRECTED DURING INSPECTION - items discarded.

Corrective Action(s): All yams discarded today due to the unsanitary manner in which they were stored. Storing food in rodent feces is completely unacceptable.

Observation: The following items need to be addressed as they are the unsanitary storage of equipment:
- Tub of dishes and equipment found on floor across from grill area.
- Cutting board in storage room stored on RAT TRAP

Corrective Action(s): Under no circumstances is ANY equipment to be stored on the floor or ON RAT TRAPS. Facility has a serious rodent infestation and nothing is to be stored on the floor as it could be contaminated with rodent feces.

Observation (CORRECTED DURING INSPECTION): Upon arrival of Health Inspector bleach sanitizer was reading 0ppm Chlorine. CORRECTED DURING INSPECTION - Bleach was added to chlorine sanitizer.

Corrective Action(s): Ensure sanitizer solution is made with adequate amount of bleach daily.

Observation: Lack of paper towels at every hand washing sink including staff washroom. Health Inspector had to search for paper towels which are located throughout on surfaces.

Observation: Rodent feces found throughout on floors. Facility needs to be cleaned and floors to be sanitized with 200ppm Chlorine. Pest Control invoices reviewed. Most recent report from April 2018. Facility lacking Pest reports for month of May. Staff indicated the company has changed.

Corrective Action(s): Increase number of traps in facility.
Seal any holes.
Keep back door closed.
Locate pest control records - ensure facility is still being serviced twice monthly.

- Storage room requires complete cleaning. Area is extremely cluttered and rodent droppings observed in area.

- Food left out overnight on dishes. Under no circumstances is ANY food or dirty dishes to be left overnight as facility has SERIOUS RODENT INFESTATION

- Rodent droppings found throughout
- Food left out from previous night

- Storage room needs to be cleaned and organized

- Ice machine and scoop in good sanitary condition
- Raw meat stored away from Ready to Eat food items

Staff had indicated that HIRAME was served on the weekend. Currently facility is not serving HIRAME as they are "out". HIRAME is NOT to be served raw unless a pathogenic kill step has occurred. Correction Order remains from May 2011. Staff unable to locate Invoices for fish today. Find all invoices for fish. These invoices are to be kept onsite.

Hirame require a pathogenic destruction step (either cooking or freezing). It is the responsibility of the restaurant to ensure the safety of food served to the customer.
The facility must find a supplier that will provide the destruction of the potential parasites/pathogens of the fish species, cook the fish species or stop serving the species.
Immediately the facility must cease serving the HIRAME species without a pathogenic destruction step. The Correction Order from May 2011 stands. Tickets will result if facility is found to serving raw HIRAME to customers without a pathogenic kill step in the processing of the fish.
