Observation (CORRECTED DURING INSPECTION): Hand washing station at front service area upon entrance not supplied with paper towel. Staff placed paper towel at the dispenser during inspection.
Corrective Action(s): Maintain all hand washing stations adequately supplied with hot and cold running water, liquid soap and paper towel at all times to promote proper, frequent hand washing by staff. Remind staff that glove us IS NOT a replacement for proper hand washing.

Observation: Multiple drain/fruit flies and few house flies observed on site during inspection (in service, dishwashing and preparation areas). UV fly trap observed in place in back preparation area. Sticky fly trap observed in place near the handwashing station in the preparation area. No traps observed in place in the service area. Professional pest control company is contracted.
Corrective Action(s): Contact pest control company to assess and apply traps/treatment as necessary. Maintain the premise in a sanitary manner to prevent fly entry/harbourage/breeding on site. Keep all food covered to prevent potential contamination.

Observation: Food premise observed in unsanitary condition. The following areas are observed with food debris build-up:
- All floors in the service area
- Floors between ovens
- Cupboards in the donut preparation area
- Floors along the walls in the dry storage area
Dirty still water observed in the cracked tile in front of the walk-in freezer.
Corrective Action(s): Clean and sanitize the aforementioned area to remove food debris. Conduct regular cleaning and sanitizing of all floors, walls and surfaces (as per approved sanitation plan) to prevent food debris build-up and to maintain the premise in a sanitary condition.

- Operating permit posted in a conspicuous location on site
