Observation: Ceiling panel exposed in the back kitchen. Small hole in floor noted in bottom of door hinge entering back kitchen.
Corrective Action(s): Replace ceiling panel so it is not exposed. Fill hole so food debris will not collect in this area when cleaning.

Observation: A few ceiling lights are out in the back. Handle for one of the three chest freezers is broken.
