Observation (CORRECTED DURING INSPECTION): 1. Bubble tea pearls observed at 28 C stored at room temperature for over 2 hours.
2. Bean sprouts held in water at room temperature observed at 16 C. At room temperature for 1 hour.
Corrective Action(s): 1. Pearls were discarded. Bubble tea pearls are a potentially hazardous product if kept at room temperature could allow for bacteria to grow. Keep bubble tea pearls:
a. Store at 4 C or below or
b. Store at 60 C or above, or
c. hold at room temperature for no more than 2 hours. Label the pearls with the time it was placed at room temperature then discard after two hours.
2. Sprouts were placed in walk-in cooler at 4 C or below. Ensure all sprouts or potential hazardous food is stored at 4 C or below under refrigeration or in a 50/50 ice bath maintained at 4 C or below.

Observation (CORRECTED DURING INSPECTION): Rice scoop by rice cooker observed in 26 C water held at room temperature.
Corrective Action(s): Scoop was removed and washed in dishwasher. Ensure the rice scoop is stored in a 50/50 ice water mixture maintained at 4 C or below or held in water at 60 C or above to prevent the growth of bacteria.

Observation: 1. Cups with no handles observed stored in dry good bins of sugar and salt in storage area.
2. Washed greens in walk-in cooler observed uncovered stored below fans.
3. Box of raw chicken observed stored on floor in walk-in cooler.
Corrective Action(s): 1. Remove all cups with no handles from dry good bins. Ensure scoops with handles are used to scoop dry goods from bins.
2. Ensure all food that is not cooling is covered to prevent cross contamination.
3. Ensure all foods and food contact surfaces are stored 6 inches off the floor to prevent cross contamination.

Observation: 1. Rodent droppings observed in upstairs storage area by traps under shelving.
2. Fruit flies observed in front area under hand sink.
Corrective Action(s): Remove rodent droppings wearing gloves and a mask. Clean and disinfect floors under the shelving where rodent droppings were found. Contact pest control company. SEND a copy of the pest control report to the undersigned for review.

Observation: 1. Excessive dust build-up observed on ceiling above kitchen area.
2. Pool of grease observed on the floor, inside a broken floor tile by the frier against the wall.
3. Excessive grease build-up observed on hood filters above grill.
4. Excessive grease build-up observed in the vent of the reach-in sandwich cooler.
Corrective Action(s): Clean the floors, filters, vents, and ceilings to remove any long standing grease and dust to prevent the attraction of pests.

- Fly trap functioning
- Quat sanitizer available at 200 ppm in kitchen and front area. Cloths stored submerged in sanitizer solution.

- CORRECTED DURING INSPECTION: No staff health checks being conducted. Manager started drafted up written health checks during inspection. ENSURE a written staff health check is conducted daily and kept on file for 30 days.

- Covid-19 signage posted on front doors
